Certain scouting activities, including summer camp programs, require that each participant (scouts and adults) provide a current health form. The health form should be completed and provided to the camping coordinator (or other other designated leader) at least 30 days prior to the event.
Cub Scout outings are generally not over 72 hours in duration and therefore do not require “Part C” and a doctor visit. When submitting your health form, include a copy of your health insurance card if applicable. Also make sure to identify the last date of each immunization.
